Early Childhood Asthma Program (ECAP)
The Community Medical Centers (CMC) Early Childhood Asthma Program (ECAP) screens, diagnoses, treats and educates families of children ages 0-5 who have asthma, reactive airway disease, recurrent wheezing, and positive family history of asthma or risk factors for asthma. The program also provides education to agencies working with children 0 - 5, providers, and works collaboratively for community asthma education and advocacy.

Nurse Liaison
The Nurse Liaison Project (High Risk Infants) provides public health nurse liaisons to Community Regional Medical Center and Children's Hospital of Central California focused on serving medically at-risk infants admitted for tertiary level neonatal and pediatric special care. The public health nurse liaisons work with hospital staff, families and health department program staff to bridge gaps in service and support infants and their families through the transition from advanced specialty care to community care.

Child Care Health Linkages
The Child Care Health Linkages program provides health and safety technical assistance, consultation, and training to local child care providers to improve the quality of services for children in their care. The program is structured after an evidence-based model from University of California, San Francisco School of Nursing. The Public Health Nurses (PHNs) assist child care providers with incorporating best practices into their facilities and supporting efforts to implement National Association for the Education of Young Children (NAEYC) and National Association for Family Child Care (NAFCC) accreditation health and safety standards. The PHNs also support child care providers by referring children with special needs and their families to nurse case management for assessment and linkage to needed services.

Nurse-Family Partnership
The Nurse-Family Partnership (NFP) is an evidence-based community healthcare program that empowers low-income, vulnerable first-time mothers to become confident, knowledgeable, and responsible parents, and ensures that their babies have the best possible start in life. Mothers are enrolled in the program early in pregnancy and receive ongoing nurse home visits until child reaches his/her second birthday. NFP nurses support mothers by connecting them to prenatal care and preventative health practices, providing individualized guidance on specific child developmental stages, as well as assisting mothers in their maternal life course development.

Learning About Parenting (LAP)
The Learning About Parenting (LAP) program provides comprehensive intensive support services to families with infants and young children at risk for child abuse and neglect. Additional family risk and stress factors may include: parental substance abuse, family violence, parents with developmental disabilities, and families involved with Child Protective Services. LAP offers weekly home visitation services, center-based support groups, and parenting education classes to further strengthen parent-child relationships, promote child health and safety, and prevent incidents of child abuse, maltreatment and neglect.
